Kentucky State Seal: There are only 4 states in the US that can call themselves a “Commonwealth.” They are Virginia, Massachusetts, Pennsylvania, and of course, Kentucky. Although a commonwealth is not Constitutionally different from a state, it is theoretically different — the term describes the government as being authorized by the common consent of its people rather than by higher authority (previously the Brits).
